Designed to improve the transition process as a patient moves from an inpatient setting, such as a hospital or rehabilitation facility, back home. Through our Care Transitions, we work closely with patients to assist in managing their disease process or rehabilitation with a goal of reducing the risk of the patient being readmitted to an inpatient facility.
The Palliative Care program uses the expertise of both our home health care and hospice teams to ensure patients receive the right care in the right setting. Through our Palliative Care program, we are able to provide care by our palliative trained nurses and social workers while still receiving all the benefits of traditional home health. The Palliative Care program is not available in all markets that Aveanna serves.
The Orthopedic Care program provides skilled home health care for joint replacements and other operative and non-operative orthopedic injuries. Our home health orthopedic team of physical therapists, occupational therapists, and nurses deliver a specialized rehabilitative approach through evidence-based practices and clinical protocols. A plan of home health care is developed and tailored to your orthopedic needs. This plan is designed to maximize mobility and activities of daily living, allowing you to regain your highest level of independence and active lifestyle.
