How soon can I get my W-2?
- Electronic W-2s: Available in Workday by January 16, 2026
- Paper W-2s: Mailed on January 19, 2026
- Access Workday: Workday (link opens in a new window)
How do I access my W-2 online?
Once available, employees can view and print their W-2 through Workday.
How to access in Workday (portal only):
- Log in through the portal (not the app): Workday (link opens in a new window)
- Click the bubble icon (beside the bell at the top right)
- Search W-2
- Select the blue hyper link labeled My Tax Documents
- Select View/Print
Former Thrive Employees
Thrive W-2s will be available after January 31, 2026, through Dayforce
How to access:
- Log in through the Dayforce Portal
- Company ID: ThriveSPC
- Username: FirstName.LastName
- Enter your password
If you are unable to log into your account, please open a ticket via the Service Center.
- Open the menu (top left corner)
- Select Earnings
- Select Year End Forms
How do I get a prior-year W-2?
Aveanna W-2s from prior years are available anytime in Workday.
For W-2s from previously acquired companies, submit a W-2 Request Form with a valid ID. The form is available in the Service Center and must be submitted through the Service Center.
To submit a request through the Service Center, you’ll need to create an Atlassian account by entering your email address, clicking the verification link sent to your email, and then setting your password.
What if I don’t receive my W-2?
- If your W-2 does not arrive by mail, it is still accessible in the Workday Portal
- Former employees keep Workday access for 1 year after separation.
- After one year, submit a W-2 Request Form with valid ID through the Service Center.
- To submit a request through the Service Center, you’ll need to create an Atlassian account by entering your email address, clicking the verification link sent to your email, and then setting your password.
Having trouble logging in?
- Active employees: Visit the Workday Portal and select “Chat with Amelia.”
- Former employees: Open a ticket via the Service Center or call 1-833-242-4357 (Mon-Fri, 8 AM-5:30 PM ET)
Notice of Availability of Tax Form 1095-C
In accordance with the Paperwork Burden Reduction Act, effective for forms for Tax Year 2025, Aveanna Healthcare will no longer be furnishing IRS Form 1095-C automatically to all active employees unless required by state law. Instead, forms will be provided upon request.
To request your form, choose one of the following options:
Option 1: If you have access to www.aveannabenefitexpress.com you may request your 1095-C form by logging into that website and selecting the option to submit a request for a copy of your form.
Option 2: To request your Form 1095-C in writing, please send your request to include the company name of Aveanna Healthcare, your full name, mailing address, and your phone number. Mail it to WEX Benefits ACA 1095, P.O. Box 2339, Fargo, ND 58108-2339.
If you have questions or need assistance logging into your portal or requesting your form, please call the Aveanna Benefit Express Help Center at 1-833-444-0010 or email BenefitsACA1095Request@wexinc.com. Forms will be available within 30 days after a request is received.
Employees living in a state where state law requires forms to be furnished will continue to receive a form automatically, and forms will be sent in accordance with the applicable state’s requirements.
